States Tighten Medicaid Home Care Program Integrity Beyond EVV

A new KFF survey of state Medicaid home care programs, published October 5 and covered by Home Health Care News on October 6, shows that Medicaid home care program integrity work has moved well past the federal EVV checklist. All 48 responding states said they keep EVV data in machine-readable files and cross-check billed claims against that visit data. Forty-seven states also mine EVV records for unusual billing patterns. For agencies that bill personal care, waiver services, or in-home home health, matching the claim to the visit is no longer a nice-to-have. It is how states are already paying and auditing.

KFF asked about eight oversight strategies beyond the federal minimum. Forty-two states reported using all eight, including claims analysis by provider and by enrollee, annual care-plan updates, in-person assessments for authorizations, and checking claims against the care plan before payment. Twelve states already use AI tools for program integrity monitoring, and nine more are piloting or planning to. Ohio is among the states that treat a matching EVV record as a condition of payment on some waivers, so a gap between the claim and the visit can mean a denial instead of a later audit letter.

What Medicaid home care program integrity means on the ground

The same week, Virginia put a concrete EVV change into force. Effective October 1, 2026, DMAS ended the live-in caregiver exemption. Agency-directed aides and consumer-directed attendants providing personal care, respite, or companion care must log shifts with an EVV-compliant method (the fiscal employer agent mobile app or IVR). The web portal alone is not compliant. CMS confirmed Virginia could make that change without a waiver amendment. It is one more signal that states are tightening how visit time is captured, not only how claims are reviewed later.

Ops teams do not need a fraud unit to respond. They need a clear line from the published schedule to what was completed, so EVV, documentation, and billing tell the same story. CareSync® gives agencies planned-versus-completed visit visibility on one board, which is the kind of plain record those integrity checks usually start from.

Worth doing this quarter: confirm how your state matches claims to EVV, note whether payment is conditioned on a valid visit record, and run a spot check on a recent week for planned versus completed visits before claims leave the door. If your state is adding live-in EVV rules or AI claim review, those same records are what reviewers will ask for first.
